Your Visit to Mountain Optical

At Mountain Optical, we want your experience at our practice to be as simple and seamless as possible. Our Middlesboro, KY office offers flexible scheduling Monday through Friday, along with walk-in fittings of our extensive selection of designer and affordable frames and contact lenses.


Below you will find an overview of new patient details and office policies to assist you in preparing for your upcoming appointment. Office Details

Our Middlesboro practice is open Monday and Tuesday, 9:00 am-5:00 pm, Wednesday and Thursday, 9:00 am-7:00 pm, and Friday, 9:00 am-2:00 pm. We are located at 2145 US Hwy 25 E, Ste A, Middlesboro, KY 40965.

  • Preparing for Your Appointment
    • Please arrive 15 minutes prior to appointment to complete any paperwork.  You can download and print the registration forms below if you would like to fill them out and  bring them with you to your appointment.
    • Please bring a complete current medication list and a list of allergies.
    • Pupil dilation is a standard part of our eye exam. Please bring a driver if you are concerned about decreased vision or light sensitivity going home.
    • All copays for services and deposits on materials ordered are payable on the date of service.
